How do I file a workers' comp. claim?
If you have been injured at your workplace in Georgia, the first thing you need to do is complete and file a WC-14 with the State Board of Workers' Compensation. Your employer can provide you with this form; if they do not have it, contact your state or local workers' compensation office to request it.
The WC-14 will give you a chance to thoroughly describe the nature and extent of your illness or injury. Once completed, send a copy of the form to both your employer and your employer's workers' compensation insurance carrier.
